As per the provision of Birth and Death Registration,1969 every death has to be registered within 21 days of the occurrence. Application Procedure Death Certificate

For applying the death certificate follow the given steps below-

  1. For obtaining a death certificate, the applicant has to fill the application form and attach all the documents.
  2. Submit the application form to MeeSeva Center.
  3. After scanning of documents application will be uploaded by MeeSeva operator.
  4. Now, pay the required fee and provide address details for receiving the death certificate by post.
  5. The death certificate will be issued on the same day but for courier, it takes 5  days to reach.

Late Registration Death Certificate

If death is registered late then the applicant need a separate form which is mentioned below in the form of pdf. Fill all the necessary details and necessary documents given in the form. Submit the application form to MeeSeva Center and after some days registrar will issue a death certificate to the applicant.
