As per the provisions Birth and Death Registration Act,1969 it is compulsory to register every occurred in Punjab within 21 days of happening. This certificate issued only to the nearest relative of the deceased person. It helps the government to know the fact of deaths happening in their State. Registration Fee Structure

Applicant has to pay the required fee for death registration which is explained below-

  • The applicant will charge Rs.5/- if death is registered after 21 days.
  • If death is registered after 30 days then it will charge Rs.10/-.
  • If death has registered after 1 year, the applicant has to pay Rs.20/-.

Required Documents Death Certificate

Applicant has to attach necessary documents while applying for a death certificate which is mentioned below-

  1. Birth proof of the deceased person.
  2. Identity proof.
  3. Deceased person address proof.
  4. An affidavit which states the place, date, and cause of death.
  5. Applicant nativity certificate.

Application procedure – Online Method

Follow the steps to apply death certificate online.

  1. Firstly,  visit the official website of Punjab.
  2. If the applicant is not registered then click on the ‘New User’.
  3. Now login by using Username and password.
  4. Select the ”Death Certificate Application Form”
  5. Fill all the necessary details and attach the documents.
  6. Submit the application form and after verification, the death certificate will be issued to the applicant.

Offline Method Punjab

If the applicant wants to apply death certificate offline then fill the application form given below and attach all the required documents then submit it to the concerned District/Town registrar.
