Registration Fee Structure
Applicant has to pay the required fee for death registration which is explained below-
- The applicant will charge Rs.5/- if death is registered after 21 days.
- If death is registered after 30 days then it will charge Rs.10/-.
- If death has registered after 1 year, the applicant has to pay Rs.20/-.
Required Documents Death Certificate
Applicant has to attach necessary documents while applying for a death certificate which is mentioned below-
- Birth proof of the deceased person.
- Identity proof.
- Deceased person address proof.
- An affidavit which states the place, date, and cause of death.
- Applicant nativity certificate.
Application procedure – Online Method
Follow the steps to apply death certificate online.
- Firstly, visit the official website of Punjab.
- If the applicant is not registered then click on the ‘New User’.
- Now login by using Username and password.
- Select the ”Death Certificate Application Form”
- Fill all the necessary details and attach the documents.
- Submit the application form and after verification, the death certificate will be issued to the applicant.
Offline Method Punjab
